Original life insurance policy. While it’s not necessary to have the original life insurance policy on hand, it can help expedite the claim filing process if you are able to obtain it.
Death certificate. A copy of the death certificate will be needed to verify the cause of death and ensure the claim is processed in accordance with the terms of the policy.
Claim request form.
